This grading system is based entirely on the histologic pattern of arrangement of carcinoma cells in H&E-stained sections.

On biopsy samples with tertiary patterns, modification of the Gleason score to equal the sum of the most common and the highest grades keeps the Gleason grading system up-to-date and accurate.
